Step 1
Employer / submitter of record
Enter the employer's information exactly as registered with the SSA. The BSO User ID is the credential issued when the Business Services Online account was created.
Step 2
Employees & W-2 statements
Enter each employee once, then add a statement for every tax year you're reporting. Type the wage into Box 3 and the
tool auto-fills Box 4 (6.2%), Box 5 (Medicare wages) and Box 6 (1.45%) — every one of them stays editable, so
type over any box with the exact figure from that year's actual W-2. Boxes 1 and 2 are your own entries. Box 3 flags
if it exceeds the year's Social Security wage base. Nothing is fixed or zeroed; all values must reflect wages actually
paid and taxes actually withheld and remitted.
Step 3
Generate the file
Your statements are grouped by tax year and combined, oldest year first, into one submission-ready file. Validate it in AccuWage Online before submitting through Business Services Online.
Leave both blank to download the plain file. Add a password and you'll get a single locked file instead — nobody can open it without this password. Whoever files it unlocks it first, then submits the plain file that comes out, exactly as before. There's no way to recover a lost password, so keep it somewhere safe.

1. Government Non-Affiliation Disclosure
MTAK (Master Templated Automated Key) is a private, independent software service and is NOT affiliated with, endorsed by, or associated with the Social Security Administration (SSA), the Internal Revenue Service (IRS), the Department of Health and Human Services, or any government agency. "AccuWage" and "Business Services Online (BSO)" are services of the SSA; references are for interoperability only.
2. Scope of Service
MTAK assists employers and authorized payroll preparers in assembling, validating, and formatting wage data into the file format required for SSA submission. The software formats the data the user enters; it does not source, verify, or certify the accuracy of that data. The user is solely responsible for ensuring every figure entered reflects true, actual wages paid and taxes withheld and remitted.
3. No Legal, Financial, or Tax Advice
MTAK IS NOT A LAW FIRM OR ACCOUNTING FIRM AND DOES NOT PROVIDE LEGAL, FINANCIAL, OR TAX ADVICE. The tool is provided for general document-preparation support only. Use does not create an attorney-client, accountant-client, or fiduciary relationship. Consult a qualified professional regarding filing obligations.
4. Accuracy & User Responsibility
Wage reports submitted to the SSA must be true and accurate. The user represents that all information entered is correct and corresponds to actual employment and payroll records, and that any Social Security and Medicare wages reported were actually paid and the associated taxes actually withheld and remitted. MTAK does not review, submit, or file on the user's behalf and bears no responsibility for the content of any file generated. Knowingly submitting false wage information to a government agency is unlawful; MTAK is intended solely for accurate, good-faith reporting and correction.
5. Data Privacy & Handling
This tool runs entirely within your browser. Information you enter is processed locally to build the output file and is not transmitted to or stored by MTAK. The generated file downloads directly to your device; safeguarding that file and any Personally Identifiable Information (PII) it contains is the user's responsibility.
6. Optional Password Protection
If you choose to lock your downloaded file with a password, that file is encrypted entirely within your own browser before it ever leaves your device. MTAK does not receive, transmit, store, or have any way to access that password or the data it protects, at any point. Unlocking the file requires the exact password set at the time of download; there is no recovery option, password reset, or backdoor of any kind, and MTAK cannot retrieve a lost password or the contents of a locked file under any circumstances. Choosing a password and keeping it safe is entirely the user's responsibility. This feature protects the file while it is stored, transmitted, or otherwise sitting unused before submission; it does not change and has no effect on the plain, unencrypted file format required by AccuWage Online or Business Services Online (BSO) — a locked file must be unlocked back to its original plain form before it can be validated or submitted through either service.
7. Disclaimer of Warranties & Limitation of Liability
The software and all output are provided "as is" and "as available" without warranties of any kind. To the maximum extent permitted by law, MTAK's total aggregate liability for any claim arising from use of the tool shall not exceed the amount, if any, paid by the user for the specific service giving rise to the claim.
